Our Client, a leading government organisation that promotes the independence for people in the community, is currently recruiting for an experienced Social Worker in Aged Care and TACP to join their team based in Western Sydney. The Transition Aged Care Program (TACP) is a joint Australian Government and State funded program. Established in 2005, this national program was developed to assist older people regain physical and psycho social functioning after a hospital stay.
This role will involve:
Working as the Social Worker, you will respond to the emotional, psychological and social needs of clients, their family and carers and in particular how these impact on the provision of care and services in their home environment and on discharge from the program.
- Working 4 to 5 days for a 2 month contract
- Working to deliver the TACP – Transitional Aged Care Program
- Case management with case load starting at 15
- Based in the office and conducting home visits (one or two per day)
- Discharge plans
- Arranging services and support for clients
- Guardianship applications
- Counselling
- Carer support and stress management
- Assisting clients, clients’ families and other people supporting clients
- Administration with guardianship application and researching for service providers
- Case conferences
- Working in Multidisciplinary Team environment
The requirements for this role include:
- A Social Work Degree
- 2 years experience of Social work in an Aged Care setting
- Driver Licence is essential for this role
- TACP experience preferred but not essential

- A number of probity checks are required including holding a current working with children and undertaking a National police checks.
