Purpose of the Clinical Nurse Specialist

As a Clinical Nurse Specialist at Achieve Australia, your primary goal is to work with a clinical team to embed good practice in the delivery of disability support with a focus on disability care, aged care and mental health. Role Responsibilities:

As a Clinical Nurse Specialist, you will need advanced clinical skills and expertise in order to address a wide range of health issues. Client advocacy and regulatory compliance are an integral part of the services we provide.

  • Conduct clinical induction for support workers and assistant in nursing (AINs), including delivery of medication training and onsite orientation.
  • Provide support to the clinical support inbox, addressing general clinical inquiries and supporting the coordination of training sessions.
  • Assist with new client transitions and organisational redesign activities, providing onsite training and clinical practice support.
  • Support the development of client-specific health care plans and complex care plans as required.
  • Facilitate professional development pathways for senior support workers, focusing on high-intensity daily personal activity skill development and competence, with particular focus on disability, aged care and mental health.
  • Provide individual coaching to clinical staff on health care plans, client assessments, hospital visits, report writing, and clinical supervision.
  • Support the Clinical Nurse Educators in performing site-specific clinical training gap analyses on a quarterly basis, offering recommendations for improvement.

To be successful in this role, you will need:

  • Current Registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) administered through the Australian Health Practitioner Regulation Agency (AHPRA).
  • A minimum of 3 years post basic registration experience, preferably in disability or aged care and evidence of ongoing professional development.
  • Well-developed interpersonal communication skills both written and verbal.
  • Demonstrated ability to work within a multidisciplinary team and autonomously as required
  • Effective time management and ability to manage competing workloads within a complex environment.
  • Experience in providing education, mentoring and support to staff in the workplace.
  • Knowledge of disability, aged care and community aged care services and NDIS Quality and Safeguarding Practice Standards.
  • Computer skills that support effective use of email, outlook calendars, Microsoft Office and client record systems
  • Ability to travel to various locations across Greater Sydney.
