The Department

Home, Acute and Community is a clinical program across Alfred Health. The program plays an integral role within Victoria’s health care system, supporting people to maximise their health, independence and functioning, and minimise long-term care needs. The Home, Acute and Community program oversees a diverse group of services promoting effective seamless care, from acute care back into the community. These services are provided in people’s homes, community centres and in hospital when indicated.

The Role

You will work within the supportive, flexible and dynamic Alfred Health Home Care Packages team, based at Caulfield Community Health Service. Home care packages support older people to live independently at home and in their local community for as long as possible, optimising their health and well-being in accordance with their needs and goals.

You will be an experienced Registered Nurse or Grade 2 Allied Health Professional (Social Worker, Occupational Therapist, Physiotherapist) or Community Development Worker who will:

  • Be responsible for the provision of case management for a caseload of complex and diverse adults and older clients.
  • Provide effective and consumer-directed assessment, care planning and care co-ordination, with a wellness and re-ablement approach.
  • Provide high quality services to clients from a variety of social, economic and cultural backgrounds.
  • Collaborate effectively with interdisciplinary team members and service providers.
  • Effectively manage clients’ package budgets.
  • Promote the Alfred Health Home Care Package program both within Alfred Health and our local community.

Qualifications and Experience Required

  • Relevant Qualification: Registered Nurse Grade 3 or Allied Health discipline Grade 2 (Occupational Therapist, Physiotherapist, Social Worker) or Community Development Worker Class 2b.
  • Eligible for membership in discipline specific professional body.
  • Current Victorian Driver's Licence.
  • Experience in health and aged care.
  • Excellent communication and problem-solving skills.
  • Excellent organisational and time management skills.
  • Competent computer skills.
