Alfred Health is a leader in health care delivery, improvement, research and education. We are the main provider of health services to people living in the inner southeast suburbs of Melbourne, from ambulatory to inpatient and home and community-based services.
Alfred Mental & Addiction Health (AMAH) is responsible for the operation of services, which focus on people with a severe mental illness residing in the Inner South Eastern area of Melbourne.
The single most important goal of AMAH is to create an environment that facilitates clinical recovery, supports individual recovery efforts and strives to minimise service system barriers to the recovery process.
The Caulfield Aged & Liaison Psychiatry Service (Older Adult Mental Health Service) provides assessment, treatment and ongoing management for people over the age of 65 years with a mental health disorder including psychological & behavioural disturbances.
The service encompasses an acute inpatient assessment unit (Baringa), and a multidisciplinary Community Assessment Team that includes Mobile Aged Psychiatry Service (MAPS).
Baringa consists of 15 beds and is staffed on an interdisciplinary model with 24-hour nursing care.
The Community Assessment Team (MAPS) is essentially the front line of the mental health program. The inter-disciplinary team undertakes primary functions of community-based assessment, treatment and case management of older persons with mental health problems.
In this role, you will use your lived experience of mental distress and recovery to provide psychosocial support and non-crisis interventions to consumers in the acute inpatient setting and post discharge for up to 28 days.
You will also provide consumer directed tailored support to consumers, in collaboration with existing treatment plans and support structures. Working within the multidiscipline treatment team, you will identify consumers who would benefit from peer support post discharge.
