Our head office is based in Heidelberg (Northern suburbs) but we have participants based all over Melbourne.
We are currently looking for 1 full-time and 1 part-time (0.6) Support Coordinator.
Roles and Responsibilities:
- Assisting participants with their weekly plans to help them work towards their goals
- Working with participants and helping them develop self-advocacy skills and to advocate on their behalf when required.
- Assisting participants to develop and maintain positive relationships with friends, family, service providers and supports
- Coordinating activities with the participant
- Access and liaise with a number of mainstreams, community, informal and provider options
- Arrange any assessments required to determine the nature and type of funding and supports needed (e.g. assessment to determine the type of complex home modifications required)
- Be able to write effective progress reports, including those required to be sent to the NDIA
- Strengthen and enhance their capacity to coordinate supports, self direct and manage supports and participate in the community
- Providing participants with assistance to: resolve problems or issues that arise; understand their responsibilities under service agreements; change or end a service agreement
- Assisting the participant get ready for their plan review by helping them:
- Assess whether they achieved their goals and got value for money for their plan
- Identify solutions to problems experienced in implementing the plan
- Consider new goals
- Helping participants decide on what actions to take to achieve goals in relation to daily living skills, life transition planning, social participation, etc.
To be eligible for the position:
- A criminal record check completed within the last 3 months must be supplied. When an employee has lived outside of Australia for 12 months or more within the last 10-year period, an international check must also be supplied on request.
- Hold a current Working With Children's Check (mandatory)
- Currently have a tertiary qualification in a relevant discipline (at least Cert IV level in mental health, community services, AOD, youth, disability etc)
- Understanding and knowledge of the NDIS, including working within NDIS guidelines
- Previous employment/lived experience in the industry or similar role.
- Good/excellent knowledge of the disability, mental health, and/or community care sector
- Must have or apply for an NDIS Worker Screen Check
- Providing support in line with the goals in the participants NDIS plan
- Keeping accurate and up to date case notes on participants progress
- Ability to identify, understand, and report any issues or concerns appropriately to management if the need arises
- Feel confident working with people with complex needs
- Be willing to travel to the Northern, Western and South Eastern suburbs of Melbourne.
- Own and drive their own vehicle
