- Part-time (0.6 FTE / 22.8 hours per week) maximum term position to 1 June 2026, based at our Noble Park site

- Annual full time equivalent salary of $70,561 + 11.5% super will be prorated

About the role
Due to an increase in client numbers we are seeking applications from experienced Case Managers to service and meet our My Aged Care Package clients’ every day needs to a high standard. If successful, you will coordinate the implementation of the case management plan and play a vital role in ensuring that clients have access to all required services to meet their care plans.
To be considered for this position, you will be able to demonstrate the following key selection criteria:
- A Diploma in Allied Health, Social Work or Registered Nursing or similar field
- A current valid Working with Children Check (Employee) for Victoria
- A current valid Driver Licence for Victoria
- NDIS Screening Clearance
- Current COVID-19 vaccination
- Be an energetic and enthusiastic team player and contributor
- Ability to successfully manage client case load, their budgets and their roster needs
- Well-developed communication and interpersonal skills, particularly building healthy rapport with clients, carers and colleagues
- Be proficient in office computer systems and be willing to share knowledge where necessary
- A comprehensive working knowledge of the Home Care Packages Operational Manual, the Aged Care Standards, Aged Care Act and an understanding of the Victorian Health Records Act
- Demonstrated ability to initiate and maintain contacts with a broad range of people and community organisations to facilitate clients' access to appropriate services
- Ability to manage time, set priorities, plan and organise your own quality work to achieve specific objectives
- Demonstrated ability to provide effective case management to clients from CALD backgrounds on home care packages
- Demonstrated ability to apply the Strengths Based Model and a Client-Centred Approach in practice with clients
- Demonstrated ability to work autonomously as well as collaboratively within multi-disciplinary teams to achieve/exceed key performance goals
- Sound and current knowledge of relevant legislation, frameworks, guidelines and insurance schemes associated with effective My Aged Care operations
- 2+ years' experience working in Aged Care is desirable
