About the Program

Our home support services help older people remain independent in their homes and communities longer. The service supports people aged 65 years and over and Aboriginal and Torres Strait Islander people aged 50 years and over who are experiencing difficulty with everyday tasks.

About the Opportunity

As the Aged Care Coordinator, you will play a pivotal role in enhancing the quality of life for our consumers. Your responsibilities will include developing and reviewing personalised Care Plans, ensuring compliance with Aged Care standards, providing effective supervision and support to staff, and maintaining up-to-date administration and financial records.

Your work will involve Home Care Packages (HCP), the Commonwealth Home Support Program (CHSP), and the Transition to Care Program (TCP) fee for service work and or any new programs that emerge from the national Aged Care reform process.

To be successful in this role you will have:

  • Approved qualification in Human Services/ Health / Community / Disability or equivalent and extensive experience working within a similar role
  • Experience in planning, co-ordination and delivery of services to older people who live at home including care assessments and the development and review of care plans
  • Working knowledge and understanding of Aged Care related reforms, legislation, compliance requirements, quality standards, the role of the Aged Care Quality and Safety Commission and the My Aged Care (MAC) portal
  • Demonstrated effective staff supervision, mentoring and support skills
  • Ability to work respectfully and collaboratively with Aboriginal and Torres Strait Islander young people, their families, communities, staff, and Aboriginal controlled organisations.

Please note: It is a requirement that prior to commencing in this role, you must provide 100 points of identification to confirm Australian citizenship or eligibility to work in Australia; possess a Working with Children (Ochre Card), a current National Criminal History Check (within 3 months) and a valid NT driver’s licence.

Experience of working closely with and having a strong understanding of Aboriginal & Torres Strait Islander culture would be considered an advantage.
