A collaboration between Anglicare NT and YWCA of Darwin, the Youth Housing Options and Pathways Program (YHOPP) provides accommodation to 15–19-year-olds who are homeless or at risk of homelessness. YHOPP provides outreach support including:
As the Case Manager you will provide intensive outreach and case management, helping them transition from OOHC, youth justice, or health facilities into stable housing. Your role includes developing strategies for independent living, strengthening family relationships, and offering culturally safe, trauma-informed support. You'll also provide information, referrals, advocacy, and group work to meet client needs and raise community awareness about youth homelessness.

Please note: It is a requirement that prior to commencing in this role, you must provide 100 points of identification to confirm Australian citizenship or eligibility to work in Australia; possess a Working with Children (Ochre Card), a current National Criminal History Check (within 3 months) and a valid NT driver’s licence.
Experience of working closely with and having a strong understanding of Aboriginal & Torres Strait Islander culture would be considered an advantage.
