Position Summary
The Capacity Building Practitioner will work within the framework of the National Disability Insurance Scheme (NDIS) to support individuals with disabilities in achieving their personal goals and enhancing their functional capacity. This role involves delivering therapeutic interventions, developing individualised support plans, and collaborating with clients, families, and multidisciplinary teams.
Key Responsibilities
Assessment and Planning:
- Conduct comprehensive assessments to identify clients' needs and goals.
- Develop and implement tailored support plans aligned with NDIS guidelines.
Therapeutic Interventions:
- Deliver evidence-based therapeutic interventions to enhance clients' skills and independence.
- Monitor and evaluate the effectiveness of interventions, making adjustments as necessary.
Capacity Building:
- Facilitate skill development in areas such as daily living, social skills, communication, and community engagement.
- Empower clients to advocate for themselves and navigate the NDIS system.
Collaboration:
- Work closely with families, caregivers, and other professionals to ensure coordinated and holistic support.
- Participate in case conferences and interdisciplinary team meetings.
Documentation and Reporting:
- Maintain accurate and timely records of client progress and outcomes.
- Prepare reports for NDIS funding applications and reviews as required.
Compliance and Quality Assurance:
- Ensure all practices adhere to relevant legislation, policies, and ethical standards.
- Contribute to quality improvement initiatives within the organization.
Qualifications and Experience
Education:
- Degree in Social Work, Rehabilitation Counselling, Occupational Therapy, Psychology or a related field.
Experience:
- Previous experience working with individuals with disabilities, particularly in a capacity-building context.
- Familiarity with NDIS processes and guidelines is highly desirable.
