We are seeking a Psychogeriatric Case Manager to join our vibrant team.

In this role you work as part of our multidisciplinary Aged Care Unit to provide support to older people experiencing mental health symptoms, with aged related health conditions.

You will be responsible for providing client support through evidence based assessment, case management, psychosocial interventions and counselling within the community, residential care and hospital settings.

You must have extensive experience working with older people with chronic disease, dementia and or mental health concerns, current AHPRA registration and an ability to travel to remote regions.
