About the Role

The Care Coordinator’s key role function is to (1) coordinate Access, Connection and Support Services for clients and staff seeking assistance from the Support Services team; (2) coordinate support services for NDIS participants seeking assistance from ATSICHS. The role will assist all parties in engaging Support Services while building capacity and enabling participants to live more independently and be included in their communities. (3) Promote, educate and facilitate knowledge transfer regarding the NDIS to ATSICHS community.

Duties

  • Maintain accurate and detailed case notes of all engagement with clients and other stakeholders. 
  • Provide ongoing Coordination of support in line with their NDIS plans. 
  • Ensure that SAs are accurate and provided in a timely manner.
  • Plan and assist with ongoing NDIS Information and Awareness sessions for participants and community. 
  • Perform Monthly review of team tasks.

Covid-19 requirements

  • Immunisation history showing you have received two doses of a Covid 19 vaccine.

Required

  • Certificate lV or higher qualifications in Health Care, Social Assistance, Community Services or Administration. 
  • Knowledge, understanding and/or awareness of the NDIS space and the challenges the participants may face.
  • Demonstrated ability to record accurate and consistent case notes.  
  • Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, and to contribute to the development of creative and innovative services. 
  • Strong organisational skills and ability to work independently and/or as part of a team. 
  • Ability to communicate sensitivity and effectively with Aboriginal and Torres Strait Islander people. 
  • Sound knowledge of Microsoft Suite Applications. 
  • Current Drivers Licence (Qld). 
  • Current National Police Clearance or ability to obtain. 
  • Working with Children’s Card (Blue Card) or be eligible to apply. 
  • Disability Worker Screening (Yellow Card) or be eligible to apply.
  • Travelling to our other locations.

Desirable:

  • Previous experience of working with Aboriginal & Torres Strait Islander clients. 
  • Previous experience in working with the National Disability Insurance Scheme (NDIS).
