About the opportunity
We are currently seeking experienced case managers to join our team in Melbourne on a Permanent Full time basis
As the Case Manager at AusCare Home and Community Care, you will be responsible for delivering case management to our clients that is person centred and enablement focused. You will be involved in all aspects of care coordination, including administrative tasks and rostering of client services. You will develop positive working relationships within the local service centre, and across the organisation to facilitate improved outcomes for our clients.
In this role, you will act in accordance with AusCare Home and Community Care policies and procedures, program guidelines, the National Standards of Practice for Case Management and the National Code of Ethics.
Key Responsibility
- You will be supporting vulnerable people in the community through Home Care Packages and Case Management.
- Ensure clients exercise choice in decision making about their own care & the way services are delivered.
- Act as the key contact person and advocate for clients and their carers.
- Continually monitor and review client needs and ensure service provision is compliant with the HCP guidelines.
- Ensure care is delivered safely and in accordance with the clients’ needs and goals.
About You
With experience in Case Management, you deliver quality, client focused care. Your ability to build strong relationships is second to none, and you have knowledge of various funding sources in the aged care sector.
You will also have
- 2 years experience in case management or community services
- Experience developing care plans for complex clients
- Experience in aged care case management
- Qualifications in Social Work, Psychology, Allied Health profession or Community Services / Case Management or university graduate degree in public health
- High level of computer systems proficiency, including data entry
- Strong prioritisation and time management skills.
- Care management, assessment, care planning, coordination and conflict management skills.
- A current and valid driver's licence.
- Required to maintain a current First Aid certificate
- Required to hold a working with children permit
- Required to provide a satisfactory national police check
- Provision of a satisfactory pre-employment medical and fitness report
- Willingness to travel across the Melbourne Metro region for Client visits
- An additional language is desirable but not necessary
- NDIS Workers Screening Check is mandatory for this position
- Evidence of Covid-19 Vaccination required
Job Types: Full-time, Permanent
Salary: $65,000.00 – $85,000.00 per year
