Awarded a contract to deliver foster care services in the Perth area, the Foundation collaborates closely with Karla Kuliny Aboriginal Corporation to provide trauma-transformative therapeutic services, making significant strides in improving care outcomes for children and young people.

About the Role:

As a Care Coordinator, you'll support children and young people in the OOHC program by:

  • Collaborating with carers, families, and service providers.
  • Developing and implementing individual therapeutic care plans
  • Managing cases using a trauma-informed approach.
  • Facilitating external referrals as needed.
  • Undertaking some weekend events and after-hours roster work.

Key Requirement's:

We're looking for someone with:

  • A tertiary qualification in Psychology, Social Work, or a related field.
  • Experience in case management and working within the OOHC sector.
  • Strong knowledge of trauma and attachment frameworks.
  • Exceptional interpersonal and decision-making skills.
  • A current driver's licence and Working with Children Check (or equivalent).
