About the Role

Reporting to the Assessment Coordinator you will provide assessment, advocacy and short-term case management to people living in the community who are experiencing vulnerability, (including those experiencing homelessness) and make referrals to appropriate services and supports.

You will be responsible for:

  • Assessing the needs of people aged under 65 years who are needing assistance to continue living in the community, referring to appropriate supports and services.
  • Under the Care Finder Program, assisting vulnerable older people who have minimal support to access mainstream support services.
  • Conducting assertive outreach to people experiencing homelessness or at risk of homelessness.
  • Establishing networks with community organisations and promoting Council support available to residents who are experiencing vulnerability.

Key Selection Criteria

To be successful in this role, you will bring the following skills and competencies:

  • Tertiary qualifications in a health, community development, social science or related discipline.
  • Two plus years’ experience in a community-based setting and knowledge and understanding of community based programs.
  • An understanding of the needs of and issues affecting people experiencing vulnerability including people who are homeless/ sleeping rough.
  • Well-developed conflict and problem resolution skills.
  • Well-developed written and verbal communication skills, including an ability to facilitate meetings.
  • Demonstrated time management and organisational skills.
  • Demonstrated ability to work as a sole worker and as part of a team.
  • Intermediate skills in Microsoft Office applications.

Mandatory pre-employment requirements

  • Current Victorian drivers licence
  • Be willing to undergo and complete a satisfactory criminal history check
  • Provide or obtain an employment-based Working with Children check 
  • Be fully vaccinated with at least two approved COVID-19 vaccines or have proof of medical exemption, in accordance with Aged Care requirements
