About the Role:

As a relatively new provider of Home Care Packages, Bayside City Council have opportunities for experienced aged care professionals to help us build a service that keeps client needs at the centre of what we do. We optimise client wellbeing while working within the scope of the home care packages program.

The Care Manager is responsible for coordinating care for home care packages clients to support them to live at home as safely and independently as practicable. We take pride in setting our Care Managers up for success with manageable caseloads that support close working relationships with clients and enabling a focus on proactive care. 

You will join an aged care team committed and acknowledged for achieving exceptional outcomes for clients. You will also be able to spend less time travelling and more time with clients due to our small geographic area and ready access to fleet vehicles.

To be successful in this role you will bring the following skills and competencies (Key Selection Criteria):

  • Tertiary qualification in a health, allied health or a social science discipline and a minimum of two years' care management experience.
  • Demonstrated understanding of the needs and challenges facing people who are ageing and wishing to remain at home within their community.
  • Experience in delivery of person-centred care and holistic care management.
  • An ability to identify client needs and respond appropriately in an empathetic, objective, respectful and professional manner.
  • Well-developed written, verbal, conflict and problem resolution skills.

Mandatory pre-employment requirements:

  • Be willing to undergo and complete a satisfactory criminal history check.
  • Provide or obtain an employment-based Working with Children check. 
  • Have no banning orders in any care setting.
  • Be fully vaccinated with at least two approved COVID-19 vaccines or have proof of medical exemption, in accordance with Council's COVID-19 Vaccination Policy.
  • Current Victorian Drivers Licence.
