About the Role:

Bayside Care and Support is seeking a motivated officer to join the fast-paced Service Intake and Commencement team to assist the onboarding of new aged care clients.

You will be responsible for:

  • Managing the My Aged Care Portal including accepting appropriate referrals on a rotational basis.    
  • Actioning incoming referrals for services received from assessment services including completion of service priority of access, an agreed service plan including consideration of safety issues and confirmation of service fee/s. 
  • Provide a high quality customer experience through the provision of timely and appropriate responses in relation phone and email requests from older people, families and service providers.  
  • Prioritising referrals based on client needs and completing an agreed number of client referrals per week.   

Key Selection Criteria:

To be successful in this role, you will bring the following skills and competencies:

  • Demonstrated experience working with older people and people with a disability in a community setting.
  • Knowledge and understanding of community based aged care services and programs essential.
  • Well-developed communication skills enabling client needs and preferences to be effectively documented for Support Workers who deliver services.   
  • Intermediate skills in Microsoft Office applications. Experience with aged care software application would be an advantage.
  • Tertiary qualifications in a health, allied health, social science or related discipline desirable.

Mandatory pre-employment requirements:

  • Be willing to undergo and complete a satisfactory criminal history check.
  • Provide or obtain an employment-based Working with Children check
