We’re looking for a Program Coordinator for Home and Community Care Program for Younger People (HACC PYP) - Home Care in our Macedon Ranges Health team. In this role, you will be responsible for managing the HACC PYP Home Care program and enhancing the overall customer experience of our clients. You will support our home care clients to live their best life and achieve their personal goals.
Macedon Ranges Health (MRH), supported by Benetas, is an award winning, not-for-profit provider of primary health, community health, home care and residential aged care across Victoria's beautiful Macedon Ranges. A typical day will have you:
- Oversight of the HACC PYP Home Care program and will also undertake assessments for the HACC PYP clients in the Macedon Ranges Shire
- Promote consumer directed care to maximise quality of life focusing on enablement and linking support to achieve so that the client is able to achieve their goals
- Deliver excellent client service to all HACC PYP Home Care clients within the Macedon Ranges Shire to maximise client wellbeing within the home
- Achieve optimum client experience outcomes, while managing business performance, monitoring targets and quality and compliance requirements. Participate in building a positive culture within the team, contributing positively to the broader Benetas community
We’re looking for someone with:
- Tertiary qualifications in Health, Mental Health Social Work or Social Services and/or commensurate
- Demonstrable experience in case management of government funded packages, community resources, and relevant legislation in the provision of aged care services, in particular HACC PYP framework
- Knowledge and experience in best practice quality models of assessment, care plan development, implementation and evaluation, with a reablement focus
- Strong skills in client advocacy and self-directed care
- A proven ability to take a holistic approach to client needs and service delivery with an understanding of trauma informed care
- Understanding of the issues experienced by clients and their carers in the home and the differing, social, spiritual and cultural needs of clients
- Familiarity with clinical practices
- Ability to build relationships with home care clients and organizations
- Able to build collaborative working relationships
- Passionate about customer service and able to create genuine connections with others
