About The Role
Anglicare is looking for a Senior Case Manager - Youth Services to support the success of our Youth Services Team by providing counselling, mediation, group work and complex case management services to young people and their families.
As a Senior Case Manager work will be completed under limited direction from your line manager and includes you undertaking more complex high-risk cases, in consultation with the Team Lead as well as taking responsibility for establishing strong network links.
This is a Permanent Full-Time role based at Mt Druitt. It is also an outreach role so you will be required to travel to clients and between sites.
Responsibilities (not limited to):
- Ensure all services are delivered professionally and to a high standard
- Ensure individual needs of clients are met by creating a case plan with the client
- In consultation with the Team Lead undertake complex case management for high-risk clients.
- Deliver group work and workshops including participating in community capacity building activities with local schools and other services.
- Ensure a full client caseload is maintained by working within funding guideline and program targets as directed by Team Lead.
- Ensure that funding body program requirements are implemented in all areas of program practice
- Responsible for ensuring comprehensive professional case notes, reports, client files and records
- Support and mentor case managers with case management, supporting growth of skills and knowledge.
- Establishing strong network links with local services to promote referral pathways and developing strong partnerships with the church.
- Contribute to development of group work and investigation/introduction of new program material, in consultation with Team Lead.
- Ensure ongoing professional development by attending relevant training, forums, courses, and other professional development activities as agreed with Team Lead.
Requirements:
- Must hold a minimum of a Diploma level qualification in Community Services or equivalent; a tertiary qualification in Social Work, Counselling, Psychology, or equivalent is preferred.
- Must hold a NSW C class drivers licence and have own comprehensively insured vehicle that you are willing to use for work purposes.
- Must hold a WWCC and be able to pass a criminal record check.
- Minimum of two years case management experience with a strong client focus on the issue of youth at risk of homelessness.
- Previous experience in networking with other organisations to achieve positive client outcomes
- Excellent written and verbal communication skills
- Competent in the use of the MS suite of products and case management software
- Strong organisational skills with ability to prioritise work and undertake effective time management
