We have recently received accreditation to provide Home Care Packages for seniors who wish to live independently at home. We're seeking a Care Coordinator to join our team as we develop our innovative approach to case management. This is an exciting opportunity to help shape our success in delivering collaborative, person-centered care.
This role offers something truly special. This isn't just another job—as a new provider of home care packages, you'll have the unique opportunity to shape our service delivery from the ground up. You'll play a key role in creating collaborative, person-centered care that aligns with our values and makes a real difference in people's lives.
The Care Coordinator is the primary point of contact for clients, coordinating all aspects of their supports and services. The role includes assessing the client's needs, developing a care plan, coordinating with healthcare providers, and conducting regular follow-ups to ensure the care plan is effectively implemented. Their primary goal is to enhance the quality of care and promote the client's overall wellbeing.
A full-time permanent role based in Waitara, you will be required to travel to client’s homes for assessments and to oversee their care management.
