We have recently received accreditation to provide Home Care Packages for seniors who wish to live independently at home. We're seeking a Care Coordinator to join our team as we develop our innovative approach to case management. About the role:

The Care Coordinator is the primary point of contact for clients, coordinating all aspects of their supports and services. The role includes assessing the client's needs, developing a care plan, coordinating with healthcare providers, and conducting regular follow-ups to ensure the care plan is effectively implemented. Their primary goal is to enhance the quality of care and promote the client's overall wellbeing.

A full-time permanent role based in Waitara, you will be required to travel to client’s homes for assessments and to oversee their care management.

Duties:

  • Provide high quality and customer- focused services to HCP and CHSP clients, their carers and families.
  • Monitor clients to address their changing needs and/or complex health or behavioural issues.
  • Complete, review and document client care plans.
  • Build and maintain successful referral pathways for HCPs across Northern Sydney and the Central Coast.
  • Provide guidance and support to frontline staff to improve quality of care and management of client needs.
  • Create, build and maintain a positive team culture.

Essential requirements:

  • Cert III in Aged Care, Community Services or Diploma in Case Management.
  • Minimum 2 years’ experience in Home Care Case Management.
  • Ability to demonstrate knowledge and skills in caring for people living with dementia.
  • Up-to-date knowledge of Department of Health and Aged Care (My Aged Care portal) and Services Australia claiming processes.
  • Excellent interpersonal communication skills, including digital literacy and administration.
  • Current unrestricted NSW driver’s licence and access to an insured vehicle.
  • Current first aid and CPR certificate.

Desired:

  • Degree in Nursing and APRHA registration.
  • Clinical quality experience and training.
