About the role:

Our Community Support Partners use a person-centred approach to assist our aged care clients to live independently within their own homes and communities. Our goal is to ensure client dignity, choice and to be able to live happier and healthier lives.

This role is a permanent part time role working 20 hours per week, Monday to Friday between the hours of 9am to 5pm.

The role will be paid as per the SCHADS Award (Home Care) Level 3 and the KMs are reimburse for travel incurred between client homes.

What your days will look like:

  • Achieve Goals Together: Implement individual support plans to help participants reach their goals.
  • Care with Heart: Provide personal care and ensure they are living with dignity, compassion, and respect.
  • Community Connection: Assist with community participation and foster meaningful social interactions.
  • Stay Informed: Know participants' profiles what they can and cannot do and always being aware of the unknown.
  • Positive Engagement: Interact positively with participants, families, carers, stakeholders, and team members.

About you:

  • Current unrestricted NSW driver’s license and access to an insured vehicle.
  • Cert III Individual Support (Ageing)
  • A commitment to following high standards of care.
  • Current Senior First Aid.
  • Willingness to travel, Km’s paid on travel between clients.
