Are you passionate about empowering others and making a meaningful impact? Better Health Network (BHN) is seeking a driven and compassionate NDIS Support Coordinator to help NDIS participants navigate their plans and achieve their goals.

Your role as a NDIS Support Coordinator, you’ll work closely with NDIS participants, helping them make the most of their plans by connecting them to tailored services. You’ll build strong relationships, provide expert guidance, and champion the independence and wellbeing of each individual.

Key Responsibilities

  • Empower participants to understand, navigate and utilise their NDIS plans effectively. Build relationships with participants, families, and service providers.
  • Connect participants with the right services, supports, and resources.
  • Monitor progress and adapt plans to ensure positive outcomes.
  • Support participants to build confidence and skills to manage their own supports.
  • Collaborate with multidisciplinary teams to ensure holistic support.

About You

  • Experience working with people with disabilities and their support networks.
  • Understanding of the NDIS framework, processes, and support pathways.
  • Strong communication, negotiation, and problem-solving skills.
  • Ability to manage multiple priorities and work both independently and collaboratively.
  • Qualifications or relevant experience in Disability, Mental Health, Social Work, or related fields desirable.

Location: Based at the vibrant Victorian Pride Centre in St Kilda, with occasional travel to other BHN sites.
