ABOUT THE ROLE

We are looking for a 3 day per week Assessment Officer to assist in the delivery of the Commonwealth Home Support Program funded home maintenance services, supporting frail elderly people in Inner West and South West Sydney.

We are seeking a friendly, reliable team member who enjoys helping elderly residents remain independent in their own homes.

Applicants must have qualifications in Social Welfare or Community Services, experience and/or knowledge of the Commonwealth Home Support Program, exceptional organisational skills and high level communication skills.

The position is based in Belmore and will require a Driver’s licence and access to a vehicle to conduct assessments at client homes.

ABOUT YOU

You will have strong administrative, project management and communication skills. You will be able to work independently and as part of a team. Your ability to communicate with clients and help them articulate their needs will be highly valued. In addition, your attention to detail will assist us to maintain a high level of record keeping as required under the Aged Care Quality Standards and our organisational policies.

Essential Criteria:

  • Tertiary qualifications in Social Welfare or Community Services
  • Experience and/or knowledge of the Commonwealth Home Support Program and Home Care Package program.
  • Exceptional organisational skills.
  • High level communication
  • Understanding of issues impacting frail older people and their carers.
  • Understanding of Consumer Directed Care/Wellness and Reablement
  • Current driver’s licence and access to a comprehensively insured motor vehicle
  • Computer skills, including Microsoft Word, Excel, Outlook and a Client Relationship Management Database.

ADDITIONAL BENEFITS

We offer an hourly rate based on a 35 hour week which is higher than the 38 hour SCHADS Award rate. Travel kms between clients are reimbursed at .99c per km. As a not for profit, our charitable status allows us to offer employees access to salary sacrifice.

The organisation is medium sized and located in both Lakemba and Belmore.

With the team spread across these two locations, we regular provide opportunities to come together through staff meetings, training and social opportunities.
