Central Coast Community Women’s Health Centre (CCCWHC) is seeking to fill a Case Manager at our Wyong centre.
The vision of the CCCWHC is ‘A feminist health centre of excellence enabling Central Coast women access to holistic health care’.
This position is to support clients to navigate complex systems, advocating for their rights and access to services by undertaking assessments, safety monitoring, planning, advocacy and linking women with other relevant and appropriate support services.
This position is funded under Communities for Children and the stipulation is that women seeking case management must have children aged 0 – 12 and live in the former Wyong Shire.
The position reports to the Case Management Practice Lead and will be based at the Wyong Centre but may also be required to work at the Wyoming and Woy Woy centres at times.
