This is an exciting role at CHP, designed to enhance access to homelessness-related resources and referrals while supporting the continuous improvement of service delivery. Applicants with lived experience will be looked on favourably.
Reporting to the Manager, Lived Experience Programs the Homelessness Referral Officer at Council to Homeless Persons (CHP) will play a pivotal role in providing timely and accurate advice and referrals to individuals seeking support.
This role focuses on determining the appropriate response and referral. This role needs to collaborate with external stakeholders and agencies. You will also contribute to the continuous improvement of intake processes, policies, and procedures, ensuring a high standard of service delivery and responsiveness.

Key responsibilities
The key responsibilities of the Homelessness Referral Officer include:
- To provide a direct service from the Homelessness Referral Service (HRS) phone line and email inbox including provision of information and referrals, including external support services.
- To help develop an effective process on the HRS line, including documenting this process and any related policies or procedures.
- To participate in other community organisations, partnerships, and new initiatives to ensure we have effective and prioritised pathways for our service users.
- Be mindful of the cultural and social influences that each person may present with, including being sensitive to trauma and providing culturally appropriate support to Aboriginal and Torres Strait Islanders.
- Work with all consumers using a strengths and rights based approach.
- Maintain conversation records of all exchanges with consumers for each support period and enter this data into the Service Records System used by CHP and in accordance with CHP policies, procedures, and other requirements.
