About the role

This position will form part of the team at the Brimbank Local Adult and Older Adult Mental Health and Wellbeing Service. This role will also be part of cohealth's Yakeen Tharn Integrated Team Care (ITC) program which has an important role within cohealth, supporting Aboriginal and Torres Strait Islander people with chronic health conditions. The role works closely with two Care Coordinators (community health nurses) and the Aboriginal Outreach Worker to support ITC clients. The role will comprise some outreach in addition to services at the Brimbank site. With support, the Aboriginal social emotional wellbeing worker they will also lead connections with First Nations community members and networks relevant to the area.

This is an identified position, therefore only Aboriginal or Torres Strait Islander people are eligible to apply.

Your duties include:

  • Engage and build rapport with clients and other staff to deliver high quality, person centred, innovative and culturally appropriate care
  • Actively engage communities who may not traditionally access mental health services, in addition to working with individuals seeking support
  • Provision of flexible one-on one SEWB support including emotional support, development of daily living skills, recovery coaching and transition planning
  • Connecting clients to culturally appropriate services in the areas of housing, justice, food security, employment, social inclusion and community
  • Support participants to work towards achieving their goals and empowering them to genuinely participate in decision-making that affects their life.

Our ideal candidate:

  • A relevant tertiary qualification and/or significant relevant experience in the Aboriginal health, mental health, housing, AOD or welfare sectors
  • Proof of qualification to be sighted
  • A sound knowledge base and relevant experience in working with Aboriginal and Torres Strait Islander people including case management and care planning for people from diverse backgrounds experiencing hardship and/or barriers to accessing services
  • Non-judgmental attitude to people experiencing physical and mental health issues, impacting wellbeing, including substance misuse.

All appointments are made subject to a satisfactory Police Check, a Working With Children Check (WWCC), complete a Statutory Declaration, evidence of legal rights to work in Australia, and evidence of vaccination against COVID-19 (3 doses). Additionally, some roles will also require current AHPRA registration and/or NDIS Worker Screening Clearance.

Roles that are determined to be Risk Category A or B [staff that have direct and/or indirect contact with body substances and are at higher risk of occupational exposure to infections] will be required to provide evidence of immunisation assessment including Influenza prior to commencement [A] or within 12 weeks of commencement [B]. This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders for COVID-19 and legislated requirement for influenza vaccination.
