About the role

The nurse practitioner position will assess, diagnose, treat, and manage patients' health issues and promote healthy lifestyles with a strong focus on preventative health care at the Brimbank Local Adult and Older Adult Mental Health and Wellbeing Service. The role will also provide clinical leadership, high level clinical nursing, management, and governance with the practice at the Brimbank site.

Your duties include:

  • Provides advanced clinical assessment/reasoning skills and autonomous care to patients and families in accordance with Nurse Practitioner legalisation and within individual scope of practice by:
  • Prescribing medications
  • Ordering and analysing pathology and radiological tests
  • Referring patients to general practitioners and other health professionals
  • Undertaking patients care on referral from health professionals
  • Multidisciplinary meetings (MDTs) and cooperating with other team members to coordinate best-practice and tailored care of our clients
  • Proactively provides combined clinical and professional leadership and assists in the maintenance of standards or nursing practice and patient care by:
  • Actively support, communicate, and coordinate medical, nursing and allied health colleagues in their clinical performance as well as assisting in their professional development.
  • Promote cost effective use of medical consumables.
  • Proactively plan and develop a problem-solving approach to patient care.
  • Practise duty of care including meeting practice standards and accountability
  • Always maintain patient and practice confidentiality
  • Ensure clinical governance processes are in place

Our ideal candidate

  • Must meet the regulatory and professional requirements for Australia including the National competency standards for the registered nurse, Code of Ethics and Professional conduct.
  • Hold a current prescriber number and be able to apply for a provider number
  • Advanced clinical experience in the assessment and treatment of illness and injury and health promotion relevant to the role and within individual scope of practice
  • Demonstrated extensive professional and/or leadership experience.
  • Demonstrated clinical problem-solving skills including conceptual and analytical skills.
  • Computer literacy

All Healthcare workers in Victoria are required to have received three doses of COVID-19 vaccination. You will need to provide evidence before commencing employment with cohealth.
