About The Communify Navigators Care Finder Service
Communify Navigators delivers services within the Brisbane North and Brisbane South Primary Health Network (PHN) regions. The Communify Navigator program is funded to deliver a Generalist Care finder Service within a consortium partnership with MICAH Projects and Queensland Positive People (QPP).
Communify Navigators act as the lead agency within the consortium for the Brisbane North sub-region, Brisbane Inner City, and Brisbane West, reporting to Brisbane North PHN. In Brisbane South, MICAH Project is the lead agency.
About the role
- Provide leadership, coordination and guidance.
- Provide senior consultation, line management, and support to the Care Finder team, who practise assertive outreach and explore innovative ways to effectively engage with individuals in the Care Finder target populations.
- Engage participants and develop trust, culturally appropriate, trauma informed professional relationships.
- Lead, set outcomes, provide advice, and offer regular direct supervision and performance feedback to support workers and other direct reports, ensuring the integration of person-centred practices to deliver safe supports that help clients work towards their individual goals.
- Work in partnership with service providers and community groups to create a support network for participants.
Role criteria
- Tertiary qualifications in Mental Health or Social Services related fields
- Demonstrated management experience in human services sector
- Sound understanding of personal, family and social issues associated with working with older adults
- Demonstrated skills and ability to work effectively with participants, family members and service providers to identify solutions
- Excellent communication skills, particularly with individuals and their families/carers who may be experiencing significant distress
- Experience liaising with external service providers including tertiary mental health services, GPs and other community-based services and organisations
- Solid understanding and of and demonstrated experience applying trauma-informed practice principles to support vulnerable people
- High level of competency using technology
