POSITION OVERVIEW

Complete Care Specialists provide services to clients with disabilities who have complex care needs. We work with private clients and relevant funding bodies including TAC, Worksafe, Funds in Court, NDIS and the State Trustees.
We are currently looking to employ a qualified and experienced Case Manager to join our team. The role requires the CM to oversee and manage funded clients with an acquired brain injury / disability to ensure they receive the necessary support, therapy, care and treatment of the highest quality, which meets the needs of the client and their family / carer.

Services are provided statewide with offices located in Boronia, VIC.

KEY RESPONSIBILITIES:

  • Complete comprehensive assessment of client and in consultation with the treating team and funding body requirements, develop a program inclusive of the client's physical and cognitive functioning, level of care, nursing requirements, OHS and social recreational needs.
  • In consultation with the client and key stakeholders develop and document a support plan for client responding to client and carer needs.
  • Review and document progress in achieving agreed actions and meeting goals.
  • Participate in the Team Meetings that guide implementation and evaluation of client’s program objectives provided by the multidisciplinary team.
  • Develop and submit a funding request as per the funding body specific requirements.
  • Participate in NDIS planning and reviews.
  • Maintain comprehensive, clear and concise case notes for each client on Workflow Max.
  • Establish collaborative working relationships and ensure Client is accessing all entitlements and respond to gaps in service provision and source resources as required.
  • Engage in monthly supervision with Management.

  • Understand and adhere to relevant company policies and procedures.
  • Follow all safe working procedures.
  • Maintain and promote OH&S Policy, procedures and systems.
  • Maintain standards relating to quality, environment and security.
  • Ensure Privacy and Confidentiality according to the Information Privacy Act 2014 and the Health Records Act 2000.

COMPETENCIES REQUIRED

Essential

  • Bachelor Degree in relevant human services field (Social work, disability, community service work, occupational therapy, allied health or nursing etc).
  • Minimum 3 years working in a related field with preference of experience working with people who have an Acquired Brain Injury.
  • Planning and organisational skills in managing self and as well as ability to effectively problem solve and negotiate.
  • Experience in working within a multi-disciplinary team.

Preferred

  • Experience working with the relevant funding bodies including TAC, NDIS, Worksafe, Funds in Court and State Trustees.
  • A sound understanding of the needs and experiences of people who receive community care services / system.

Attributes

  • Demonstrated ability to liaise and consult with a broad range of health professionals, community agencies and organisations and families.
  • Excellent interpersonal, communication (listening, written and verbal), meeting facilitation and liaison skills.
  • Ability to maintain professional standards whilst building relationships to support client to remain living at home independently.
  • Ability to contribute to a positive team environment.
  • An understanding of and experience in the delivery of person-centered practices and complex case management.
  • The Case Managers will work within funding guidelines maintaining professional boundaries and respect at all times.
  • Time management, planning and organising their own work.
