Connected Support Coordination is a registered provider of support coordination, coming up to 3 years of providing this support to people living on Sydney’s Northern Beaches, and the greater Northern Sydney area. Connected Support Coordination is unable to meet the demand and increasing referrals for their support coordination services. Due to this Connected Support Coordination is now looking to build a small exceptional team. A Support Coordinator and/or Specialist Support Coordinator who has a strong background in support coordination, with a sound knowledge of the disability sector and the NDIS. Full-time and part-time opportunities.

Essential Criteria

  • As an Independent Contractor position, you will be required to manage all aspects of being an Independent Contractor including tax requirements, and expenses, along with providing your own computer and phone, etc.
  • Australia Business Number (ABN).
  • Public Liability and Professional Indemnity Insurance.
  • Copies of relevant qualifications.
  • 100 Points of identification.
  • Full Australian working rights.
  • Minimum 12 months experience working with people with disabilities in a Support Coordination role.
  • Specialist Support Coordinator: Bachelor degree in a health related discipline (Social Work, Psychology, Occupational Therapy or other Allied Health, and should be affiliated with the relevant professional body) and experience working as Specialist Support Coordinator.
  • Good knowledge and understanding of NDIS legislation and guidelines, and how they are applied. Including interpreting and implementing all aspects of NDIS plans
  • Excellent communication skills, both written and verbal
  • Dedicated commitment to supporting and improving outcomes for individuals
  • Valid Driver’s licence and comprehensively insured motor vehicle
  • Hold a current Working with Children Check
  • NDIS worker screening check
  • Committed to providing a person-centred and strengths-based approach
  • Commitment to ongoing professional development
  • Must live in the Sydney Northern Beaches or Northern Sydney area.
