As an Aged Care Navigator, you will be assisting the most vulnerable older people in the Cairns community in need of intensive assistance to access aged care and other supports. Our Aged Care Navigators help to identify, understand and apply for services on our client’s behalf, connecting our clients to community supports according to their needs as part of the Care Finder program.
The position is a part-time permanent position (4 Days, 30.4Hrs).
As an Aged Care Navigator you will manage assigned cases that encourage person-centred care working across clients from diverse backgrounds and needs.
This role includes engaging with a wide range of community members, community groups and organisations, and having consumers rights respected.
