<p>At Central and Eastern Sydney Primary Health Network, we focus on improving the health and wellbeing of the people who live and work in Sydney, from East to South and the Inner West. Primary health refers to the healthcare provided in the community, such as a local doctor, pharmacist or psychologist. We provide support and training to these health professionals. We help them to connect community members to other services so that everyone involved in a person&rsquo;s care can know how best to look after their health.</p>
<p>PHNs are not-for-profit organisations. We receive money from the government to provide health services that are right for the people in this area. We also work with hospitals, specialists, aged care facilities and others to improve people&rsquo;s experience when needing healthcare. We work to make it easier for people to get the best care and support when and where they need it to achieve our vision of healthy and thriving communities.</p>

<h3>About the job</h3>
<p>This position is part of a broader effort to improve the health system experience and response for people with an intellectual disability accessing primary care services. The Intellectual Disability Educator and Service Navigator is responsible for assisting to improve access to primary care and other specialised services for those with an intellectual disability and their carers. This position will help to promote referral networks utilised by primary care providers to ensure those with an intellectual disability and their carers get access to quality services that best addresses their needs. This position will also assist to identify opportunities for further collaboration that will improve care networks available for those with an intellectual disability.</p>
<h3>Key responsibilities</h3>
<ul>
<li>Assist with providing in-reach service navigation and referral support to primary care providers to improve the continuum of care and reduce service gaps for people with intellectual disability.</li>
<li>Develop and deliver evidence-based training and resources for primary health care providers to build skills and capacity in delivering health care for people with intellectual disability</li>
<li>Assist general practices and allied health providers to engage in person centred care, quality improvement activities and apply a systematic approach to the provision of patient care to optimise outcomes.</li>
<li>Participate in local inter-agency groups to increase knowledge of the local service system, build relationships and promote the project.</li>
<li>Contribute to the preparation of local needs assessments, assist with monitoring, research and reporting on program outcomes, and help implement evaluation plans for key program areas, as directed.</li>
</ul>
<h3>Selection criteria &ndash; essential</h3>
<ul>
<li>Tertiary qualifications in a health-related field and/or relevant experience.</li>
<li>Relevant knowledge, training and experience in intellectual disability.</li>
<li>Understanding of the health and disability sectors, with general knowledge of relevant legislation, regulations and guidelines.</li>
<li>Experience in facilitating educational workshops and supporting the development of quality education resources.</li>
<li>Demonstrated experience in stakeholder engagement and consultation in the primary health sector.</li>
<li>Demonstrated ability to assist in establishing and maintaining relationships and partnerships with a wide range of stakeholders including the Local Health Districts, non-government organisations, community organisations, universities, clinicians and the public.</li>
<li>Understanding of recent disability reforms including the National Disability Insurance Scheme.</li>
<li>Experience supporting the development, implementation and evaluation of community-based health promotion programs in cross cultural settings.</li>
<li>Strong organisational skills and the ability to set priorities, work effectively and achieve outcomes in complex and challenging environments.</li>
<li>Demonstrated high level written and verbal communication skills, and ability to maintain diplomacy in internal and external correspondence.</li>
<li>Comprehensive knowledge of Microsoft Office based computing applications, e.g. Excel, Word, Database, SharePoint etc; and</li>
<li>Access to a motor vehicle and possession of a current NSW drivers&rsquo; licence.</li>
</ul>
