At Central and Eastern Sydney Primary Health Network, we focus on improving the health and wellbeing of the people who live and work in Sydney, from East to South and the Inner West. Primary health refers to the healthcare provided in the community, such as a local doctor, pharmacist or psychologist. We provide support and training to these health professionals. We help them to connect community members to other services so that everyone involved in a person’s care can know how best to look after their health.

PHNs are not-for-profit organisations. We receive money from the government to provide health services that are right for the people in this area. We also work with hospitals, specialists, aged care facilities and others to improve people’s experience when needing healthcare. We work to make it easier for people to get the best care and support when and where they need it to achieve our vision of healthy and thriving communities

About the job

The Population Health and Chronic Disease Program Officer is responsible for developing, implementing and monitoring key program activities to ensure they meet agreed-upon outcomes and performance indicators.

The Program Officer will undertake tasks related to commissioning and project management, stakeholder engagement, health system integration and quality improvement in General Practice.

Key responsibilities

  • Plan and develop new population health programs, which may involve creating program specifications and procurement documentation from the tender stage through to contract completion.
  • Monitor and report on program activities, budget and outcomes, and implement evaluation plans, collect and analyse program activity data.
  • Develop project plans and timelines in priority areas as directed and in conjunction with CESPHN Leadership and Program Advisory Committees.
  • Support general practices and allied health service providers to engage in person centered care and, quality improvement activities.
  • Develop and promote referral pathways to meet the needs of our local communities, particularly in relation to chronic disease management and prevention.

Selection criteria – essential

  • Tertiary qualifications in a health-related field and/or relevant experience
  • Demonstrated experience in stakeholder engagement and consultation, ideally in the primary health sector
  • Experience in developing, implementing and evaluating population health programs and/or clinical service delivery programs
  • Strong organisational skills and the ability to set priorities, work effectively and achieve outcomes in complex and challenging environments
  • Demonstrated high level written and verbal communication skills, and ability to maintain diplomacy in internal and external correspondence; and
  • Comprehensive knowledge of Microsoft Office based computing applications, e.g. Excel, Word, Teams, SharePoint etc.

Selection criteria - desirable

  • An understanding of the primary health care environment and experience in communicating with health care professionals and consumers within CESPHN boundaries
  • Ability and willingness to facilitate educational workshops and develop quality education resources; and
  • Access to a motor vehicle and possession of a current NSW drivers’ licence.
