<p>At Central and Eastern Sydney Primary Health Network, we focus on improving the health and wellbeing of the people who live and work in Sydney, from East to South and the Inner West. Primary health refers to the healthcare provided in the community, such as a local doctor, pharmacist or psychologist. We provide support and training to these health professionals. We help them to connect community members to other services so that everyone involved in a person&rsquo;s care can know how best to look after their health.</p>
<p>PHNs are not-for-profit organisations. We receive money from the government to provide health services that are right for the people in this area. We also work with hospitals, specialists, aged care facilities and others to improve people&rsquo;s experience when needing healthcare. We work to make it easier for people to get the best care and support when and where they need it to achieve our vision of healthy and thriving communities.</p>

<h3>About the job</h3>
<p>In the 2021-22, the Australian Government committed funding to all PHNs to expand the Greater Choice for At Home Palliative Care Measure (GCfaHPC) which aims to boost palliative care coordination and integration for people who have a known life-limiting condition, by improving choice and quality of care in the home. Under the program, PHNs are developing and implementing initiatives to achieve the following objectives:</p>
<ul>
<li>Improving access to palliative care at home and support end-of-life care systems and services in primary health care and community care</li>
<li>Enabling the right care at the right time and in the right place to reduce unnecessary hospitalisation</li>
<li>Generating the use of data to support continuous improvement of services across sectors</li>
<li>Supporting the use of available technologies to support flexible and responsive palliative care at home, including in the after-hours.</li>
</ul>
<p><strong>Overarching outcomes of the role include:</strong></p>
<ul>
<li>Improving capacity and responsiveness of services to meet local needs and priorities</li>
<li>Improving patient access to quality palliative care services in the home</li>
<li>Improving coordination of care for patients across health care providers and integration of palliative care services in their region.</li>
</ul>
<h3>Key responsibilities</h3>
<ul>
<li>Provide coordination of CESPHNs Greater Choice for at Home Palliative Care (GCfaHPC) program in alignment with Department of Health and Aged Care guidelines.</li>
<li>Take a lead responsibility for the planning, development, implementation and monitoring of palliative initiatives that work to address the identified needs and gaps experienced by people wishing to access palliative and end-of-life care.</li>
<li>Work with key palliative care stakeholders to ensure an integrated approach to Palliative Care initiatives.</li>
<li>Manage project contracts for commissioned services ensuring agreed objectives are met within agreed timeframes and budget.</li>
<li>Collaborate with other internal teams (Population Health, Digital Health and Practice support teams) to optimise program outcomes</li>
<li>Comply with any reporting requirements from the DOHAC by collecting, collating, and communicating relevant data to the appropriate stakeholders and contacts.</li>
<li>Liaise with and engage with the DOHAC GCfaHPC Community of Practice sessions</li>
<li>Manage all stakeholder enquiries relating to Palliative Care and the GCfaHPC program</li>
<li>Other aged care related duties and responsibilities as delegated by manager.</li>
</ul>
<h3>Selection criteria &ndash; essential</h3>
<ul>
<li>Relevant tertiary qualification in health, palliative care, aged care, nursing, pharmacy or project management (or equivalent experience).</li>
<li>Experience in project management and the commissioning of programs and projects in a health environment.</li>
<li>A strong understanding of palliative care and end of life systems and issues as they relate to quality of care.</li>
<li>Highly developed interpersonal and communication skills with the capacity to build relationships with key external stakeholders.</li>
<li>Ability to manage complex tasks and meet deadlines in a high demand work environment.</li>
<li>Ability to work both independently and as part of a team.</li>
<li>High level of computer literacy.</li>
<li>Experience working with vulnerable and underserved populations.</li>
</ul>
