Elevate is prominent independent Support Coordination provider based in the Hunter and Central Coast regions of NSW. Working closely with our communities and collaboratively with mainstream services, we proudly deliver individualized and person centered support to our clients.

Elevate does not operate on a KPI’s or any other system of minimum case loads etc. We do not dictate your minimum case load or minimum work hours, you will. Elevate is not focused on company profits thus we can support our work family as they put their best foot forward for our clients. Since its conception Elevate has added 10 family members and has a brilliant retention rate of team members so don’t be put off by the “contractor role” phrasing. In order to get the best and do the best we also have a remuneration that is heavily skewed in the applicants favour as we seek to find the best professionals available for our clients.
Our highly skilled and well respected team work remotely using Elevates resources to deliver support to our clients. Our approach to support has lead to some strong and well established relationships within our community, and as a result consistent referrals are contributing to our continued growth as an organisation.
Elevate is an independent provider of Support Coordination. We do not deliver other supports under the NDIS framework and are therefore able to offer a non biased approach to service delivery.
The role of Support Coordinator with Elevate is to deliver Support Coordination within the NDIS framework, including Support Connection, Support Coordination and Specialist Support Coordination.
You will be using a strengths based approach to assist participants to work towards their identified goals by working together to navigate the NDIS service systems, making informed decisions and obtaining the resources and support they need.
