About the Opportunity

North and West Remote Health Ltd (NWRH) are currently seeking 2 full-time Diabetes Educators to join their passionate teams in Mount Isa and Longreach, QLD.

Reporting to the Team Leader, your primary responsibility will be to plan, deliver, and evaluate diabetes-specific outreach services to the NWRH catchment area within Mount Isa or Longreach, utilising group education and one-to-one activities as appropriate to meet individual needs.

Your day to day responsibilities will include:

  • Integrating diabetes self-management education with clinical care as part of a therapeutic intervention to promote physical, social, and psychological well-being;
  • Assisting clients to understand their personal health risks and the implications of those risks;
  • Developing comprehensive self-management plans to maximise positive health outcomes for clients;
  • Liaising with primary care and community organisations to improve the health of individuals and the community by raising awareness of healthy lifestyles, disease, and disability; and
  • Working with other Health Professionals in the overall case management of clients.

Successful candidates will receive an attractive remuneration package circa $93,926 - $108,587 (commensurate with skills and experience) inclusive of super, salary sacrificing options and accommodation allowance (up to $180/ fortnight).

A fantastic range of ADDITIONAL benefits will also be provided, including:

  • Annual leave loading of 17.5%;
  • 5 weeks annual leave;
  • Annual return flight home;
  • Relocation costs reimbursed (negotiable up to $5,000); and
  • Up to 4 weeks free accommodation, to assist in your initial transition/ relocation to Mount Isa.
