Position Overview:

The Support Worker is responsible for providing compassionate and professional in-home care and support to individuals with disabilities or elderly individuals who require assistance with their daily living activities in and around their homes. This role involves promoting independence, enhancing quality of life, and ensuring the overall well-being of the individuals in their care. The Support Worker works collaboratively with participants, their families, and other health professionals to deliver person-centred care that meets the unique needs and preferences of everyone.

Supervision:

All employees must abide by the NDIS Code of Conduct, the Aged Care Royal Commission Code of Conduct, as well as the organisation’s Code of Conduct.

A Disability and Aged Care Support Worker operates within established routines, methods, standards, and procedures while working under general guidance. They are responsible for effectively managing their time, planning, and organising their own work.

Key Responsibilities:

  • Personal Care Assistance
    • Provide support with personal hygiene, grooming, dressing, and toileting, while respecting privacy and dignity. Assist with mobility, transfers, and positioning to ensure comfort and safety.
  • Daily Living Support
    • Assist with household tasks such as meal planning and preparation, housekeeping, laundry, and grocery shopping. Promote and encourage independence in daily activities while offering necessary assistance.
  • Medication Management (If qualified).
    • Administer medications according to prescribed instructions, ensuring accurate dosage and documentation. Monitor medication supply, coordinate refills, and report any concerns to the appropriate healthcare professionals.
  • Emotional Support and Companionship
    • Foster a supportive and trusting relationship with clients by actively listening, engaging in meaningful conversations, and providing companionship. Offer emotional support and encouragement to enhance overall well-being.
  • Care Plan Implementation
    • Implement care plans developed by healthcare professionals, ensuring all goals, strategies and interventions are followed. Monitor and record progress notes, report any changes or concerns, and collaborate with the care team to adjust plans as needed.
  • Documentation and Reporting
    • Maintain accurate and timely shift progress notes of client interactions, activities, and care provided. Complete incident reports and communicate any changes in health, behaviour, or incidents to the appropriate personnel through Progress Notes.
  • Safety and Risk Management
    • Promote a safe environment by identifying and addressing potential hazards or risks. Follow proper infection control practices, maintain equipment and supplies, and ensure a clean and organised living space.
  • Duty of Care
    • The legal and ethical obligation of our Support Workers and all staff to act in the best interests of those they care for, taking all reasonable steps to prevent harm and promote well-being. This encompasses a broad range of responsibilities, from ensuring the physical safety of clients to protecting their rights, dignity, and autonomy.

Qualifications:

  • Relevant qualification in disability and / or aged care, such as a Certificate III or IV, Diploma, or Bachelor's in disability, ageing support, community services, social work or similar.
  • Experience working in a similar role within a disability, aged care, or clinical setting will be highly regarded.
  • Strong communication and proactivity highly regarded.
  • Interpersonal skills, with the ability to build rapport and trust with people, establish positive relationships with clients, families, colleagues, and office staff.
  • Empathy, patience, and the ability to adapt to diverse needs and challenging situations.
  • Knowledge of person-centred care approaches and understanding of the Charter of Aged Care rights and NDIS participants rights.
  • Ability to follow company procedures, guidelines, and care plans and maintain client privacy and confidentiality is a must.
  • Ability to perform tasks that may involve manual handling i.e., pushing, pulling, holding, bending, lifting, and assisting with mobility.
  • Valid driver’s license, access to a reliable vehicle, car registration and car insurance (if and when required for transportation duties).

Other key information.

  • Shifts are a minimum of 2 hours per participant with multiple participant.
  • Travel is required between each client.

In addition to the outlined tasks, you will be responsible for undertaking other job-related assignments as requested by the client and/or the person(s) you report to and as required for the growth of both the role and the business.

  • Pay: $30.33 – $36.89 per hour
  • Expected hours: 6 – 45 per week
