Fronditha Care is a longstanding Aged Care Provider committed to delivering services to Australian elders of Culturally and Linguistically Diverse Backgrounds. We focus on providing unique services both in residential and community care for elderly people of Greek speaking background in Victoria and of multilingual background in New South Wales.

An opportunity has arisen for a highly motivated and experienced individual to join our Community Services Division as a Care Advisor. Based at our modern Support Hub in Mulgrave with mobile and flexible working arrangements, this is a Full Time position and requires a lateral thinker who thrives on ‘thinking outside the square’.

Requirements

The Care Advisor's roles and responsibilities include:

  • Manage a portfolio of clients, including their care needs and funding
  • Work within Fronditha Care’s Consumer Directed Care Model of Service.
  • Provide support and leadership to care staff through peer support, supervision and training.
  • Ensure service delivery meets consumers’ needs, HCP Operational Manual, The Aged Care Act 1997 and Fronditha Care’s quality assurance goals.
  • Participate in the Community Services after hours 'on call' service on a rotating basis.
  • Ability to work independently and as a member of a dynamic team.
  • High level of motivation.
  • Problem solving skills.
  • Have a 'can-do' attitude

To be considered a successful candidate, you will possess the following:

  • Qualifications in Social Sciences, Nursing, Allied Health or equivalent,
  • Minimum 2 years’ work experience in the field,
  • Understanding of the aged care and dementia care,
  • High degree of computer literacy
  • Demonstrated experience in managing care packages in accordance to contractual requirements and government programs and guidelines.
  • Ability to speak Greek is highly desirable as well as a current driver’s license with access to own registered vehicle (staff of community services also have access to pool cars during business hours).
