ABOUT THE GLEN FOR WOMEN
The Glen for Women’s program is designed to treat a client in a holistic manner by addressing all of their issues. This means that we treat the person as a whole (spiritually, mentally and physically). We don’t just treat their addiction, we also look at things in their life that may be causing the addictive behaviors.
The Glen for Women is a 20-bed residential rehabilitation centre located at Wyong Creek. We have recently purchased a 10-bed transition house at Wyong, which requires this role.
Our program is about empowering people to take control of their lives, to live a good, fun life and to become active members of their families and the community as a whole.
CASE MANAGEMENT RESPONSIBILITIES
- Plan and provide holistic and individualized case management to up to 10 transition clients, to help them achieve their goals which can include:
- Obtain qualifications and education
- Housing
- Legal matters
- Independent transport
- Employment
- Budgeting/financial management
- Case management at The Glen requires managing strong & effective relationships with service delivery partners such as employment services, employers, training providers, housing providers, DCJ and other government departments.
- Supporting clients to plan their weekly routine, to achieve their goals
- Understand and implement exit planning
- Undertake routine client outcomes measures (COMS) and progress monitoring with client.
- Chair a client transition group meeting fortnightly.
- Ability to plan and facilitate psycho-educational groups.
- Monitoring the property and ensure house rules are abided by, and the cleanliness of the property is maintained.
- Support the clinical team to make informed client decisions, based on facts.
- Provide aftercare support and following up with clients after they’ve completed the program.
- When required, collect random and scheduled breath tests and urine analysis from clients.
- Crisis management: Work collaboratively with management team in times of crisis, including communicating to staff, clients and the Board.
- Assist clients exiting the program under a disciplinary discharge
ADMINISTRATION RESPONSIBILITIES
- Basic project management skills will be required in the early stages of establishing The Glen for women transition house.
- Ensure case notes based on observable behavior and facts.
- Maintain client case notes, records and reports as required including appropriate and timely clinical documentation, recording of treatment results, care plans, outcomes and recommendations in the client file on the client management system.
- Complete shift reports in a timely manner.
- Actively contribute to staff meetings, team development activities, performance initiatives and service development activities.
- Understand and complete program risk assessments, including one-pagers for new activities.
ABOUT YOUR ROLE
The Transition Case Manager oversees clients throughout the transition program, ensuring their needs are understood, documented, and addressed by coordinating with appropriate services and stakeholders. Responsibilities include managing case files and entering client data into various online reporting tools. The Case Manager also collaborates closely with the counselling staff at The Glen For Women.

REMUNERATION
The Glen remunerated according to the SCHADS award with the level to based on your experience & qualifications and can be negotiated.
