About Community Interlink :

Community Interlink is a consortium model consisting of 18 local Public Health services. Community Interlink provides Aged Care Case Management and Support Coordination to NDIS recipients.

Community Interlink pride ourselves on supporting local people by local people. This role is located in Mansfield and is supported by the Seymour and Benalla teams.

The servicing region will cover Mansfield and surrounds supported by the Seymour Program Manager. The role is a GV health role that is co-located with Mansfield District Hospital Community Home Support Program staff.

About the department:

Community Interlink is an equal opportunity employer and consists of about 50 staff througout our the Hume and surrounding Regions. These staff are situated in various locations accross the servicing area. This role will be predominately servicing Mansfield consumers to utilise their Home Care Package funds to maintain independence and wellbeing and NDIS participants to understand and utilise their NDIS plans. The Community Interlink team is a supportive and inclusive environment that will support your learning and development.

About the role

The role is predominently an Aged Care Case Manager role with potential for growth into NDIS Support Coordination depending on the successful applicant. You will support Home Care Package recipients to understand their funding and source supports to meet their individual needs. you will help them to maintain their independence and safety in their homes for as long as possible by mitigating risk and supporting access to help support ageing well. As a Support Coordinator you will be the interpreter between your participants and the NDIS.

You will have access to to a team of highly skilled and supportive Case Managers and Support Coordinators to ease you into this role and guide you through it.
