About the team:

Community Interlink is a passionate, responsive, caring and client focused team, who assist some of most vulnerable members of the community through their Aged Care and NDIS funded supports.

About the role:

Community Interlink provide access to support services for frail older people, people with disabilities and their carers. The funding used in the program will help to provide in home supports for consumers to maintain safety, choice, dignity and independence in their home for as long as possible.

This position forms part of the Case Management / Planning team responsible for providing planning and case management services to our aged care package recipients and NDIS consumers. Case Management is predominantly a technical role confined to supporting clients to utilise their funds within the guidelines effectively. This includes, Care Planning, Assessment, Coordination of Supports, goal setting, monitoring effectiveness and formal reviews.

To be successful at the role you will need to be able to think outside the box and have an understanding of local services available. You will have an understanding of the social model of health and support people to have access to their local community.

You will have the opportunity to work collaboratively with other health professionals to maintain and build upon the capabilities of your clientele.

You will work with people to identify and meet their goals in this consumer directed care model.

We are looking for:

  • A Tertiary qualification in an appropriate Health and/or Human Services field, or extensive, relevant experience
  • Sound experience of the knowledge and skills required in working with frail older people, people with disabilities and their carers.
  • Well-developed interpersonal and communication skills, including the ability to initiate and maintain contacts with a broad range of service providers and community agencies.
  • Knowledge of the Case Management approach to service delivery in a community setting.
