About the Role

We’re looking for a highly motivated individual to join our team as the Program Coordinator or Client Care. In this role, you will manage our Client Care Metro team provides frontline crisis support to individuals and families experiencing homelessness. You will oversee triage, assessments, referrals, and outreach services and practice trauma-informed care and solutions-focused outcomes.

Key responsibilities of the role include:

  • Ensure that the triage function identifies and responds to the level of need and vulnerability of clients.
  • Oversee a team that provides the appropriate level of assessment and referral, so clients are supported and provided with suitable outcomes in a timely manner and within the funding agreement.
  • Ensuring that client needs are met within funding guidelines and that compliance and service standards are met in line with legislation, policy, and funding agreements.
  • Work with other relevant organisations (partner agencies) to enhance outcomes for individuals and families in the geographical area, including neighbouring Local Government Areas (LGAs).
  • Support the Haven Home Safe operations team in implementing continuous improvement initiatives to meet business goals and key objectives.

About You

We’re looking for an individual with:

  • Proven experience in team management and leadership.
  • Demonstrated experience working in the housing solutions industry including triage, intake assessment and planning, financial support, referral, and outreach services.
  • Extensive knowledge of homelessness services and community support systems.
  • Experience with CRM systems and data management.
  • Ability to work collaboratively with diverse teams and external partners.
  • Tertiary Qualifications in a relevant field (Social Work, Community / Human Services, etc.) is highly desirable.
