Position Purpose

The purpose of this position description is to outline the roles and responsibilities as a disability support worker to assist our participants to complete tasks, build their capacity and engage with the community.

KEY ACCOUNTABILITIES AND RESPONSIBILITIES

  • Comply with the NDIS Code of Conduct to support participants in ways that are respectful and inclusive.
  • Apply principles of positive behavior support when supporting participants to implement their behavioral support plan.
  • Provide personal care, such as showering, dressing or other duties, responding flexibly to participant preferences.
  • Assist with community access including but not limited to assisting participants in shopping, attending social events and appointments.
  • Ability to work independently and/or within a team environment.
  • Provide services to the participants as per the Participant’s support plan and/or health care plan.
  • To maintain Participant confidentiality at all times.
  • To participate in training programs as required
  • Ensure progress notes and other documentation are clearly documented and signed at each shift.
  • Comply with Holistic Care SA policies and procedures.
  • Ability to work within the Work Health and Safety (WHS) Principles including safe manual handling procedures.

ESSENTIAL MINIMUM REQUIREMENTS

  • Certificate III in Individual Support (Ageing or Disability) qualification
  • Current NDIS Work Worker Check
  • Current Senior First Aid Certificate
  • Current CPR Certificate
  • NDIS Worker Orientation Module
  • Current Manual Handling Certificate
  • Mandated Notification Training and Working with Children’s (WWC) Check (if working Certificate III in Individual Support (Ageing or Disability) qualification
  • Current NDIS Work Worker Check
  • Current Senior First Aid Certificate.

STATEMENT OF KEY OUTCOMES AND ASSOCIATED ACTIVITIES

The performance of the disability support worker will be appraised by the Participant Services Manager. The key areas of evaluation will be:

Participant focus

  • Meet the participant's needs in a timely and professional manner.
  • Support the participant to achieve their goals.
  • Report any concerns you may have noticed during the shifts.

Communication and teamwork

  • Listens effectively, conveys and receives information and direction.
  • Well written and verbal communication with the participants and office staff.
  • Work harmoniously with all staff and demonstrate support for other team members, assisting them to achieve service delivery excellence.
  • Develop a good understanding of the services delivered by Holistic Care SA

SUPPORT WORKER DUTIES

The role of the Disability support worker varies depending on the participants’ support needs and services requirement, however general duties that are expected of the position including:

  • Provide personal care assistance as required, including but not limited to:
    • Assistance in personal care including feeding, toileting, bathing and grooming, dressing/undressing, hair routine, dental care and where applicable makeup application.
    • Assistance in toileting, including assisting Participant on and off toilet/commode chair, and assisting with hygiene if required. Some Participants may require assistance with removal and replacement of catheter bags or incontinence aids.
  • Medication – administering medication or other medical/special procedures where required. Disability support workers must not administer medication unless completion of relevant medication training and follow as per the support plan.
  • Transporting participants in vehicles to various locations (such as shopping, attending appointments and social events) or assistance using public transport.
  • 2. Provide domestic assistance, including but not limited to:
    • Cleaning floors – vacuuming, sweeping, mopping
    • Cleaning bath, shower, vanity basin, and toilet
    • Cleaning kitchen – wiping benches, cupboard doors, cleaning sink, washing and drying dishes, emptying/loading dishwasher, cleaning stove top/oven/fridge, empty and clean bins, (not the big wheelie bins), defrosting of refrigerators
    • Laundry – washing personal clothing and linen/towels and hanging out clothes or putting in dryer, removal of dry clothes from line or dryer, folding and storing, ironing
    • Dusting and tidying
    • Making beds, changing linen
    • Meal preparation – cooking or preparation of meals or heating of frozen meals or Meals on Wheels
    • Shopping on behalf of Participant, making lists, unpacking and storing bought items
    • Assisting with the care of pets and care of plants, indoor and outdoor
    • Sweeping paths, light gardening
  • Apply appropriate manual handling techniques where required when there is a need to transfer the participant, including transfers using hoists or other manual handling aids.
  • Establish effective communication with the participants using the appropriate communication methodology.
  • Accurately complete administrative tasks as required, including:
    • Participant progress notes
    • Communication books
    • WHS, incident reporting form and ABC Charts
    • Medication charts/form
    • Kilometer claims
    • Reading and responding to emails
    • Other required documentation
    • Perform necessary tasks and ensure the premises are kept clean and tidy.
    • Attend and contribute to any relevant meetings as required and requested by Holistic Care SA.
    • Attend training and complete necessary assessment to enhance knowledge and skills and comply with the training requirements.
    • Where required disability support workers will provide overnight support in the Supported Independent Living (SIL) Homes, this service involves staying overnight in the participant’s home and may include any or all aspects of personal and home care services.

Personal Abilities/Aptitudes/Skills

  • A passion in providing support to people with disability where the whole person is supported enabling them and equipping them to live an independent lifestyle.
  • Demonstrated capacity to work with honesty and integrity.
  • Ability to communicate effectively and appropriately with people with disabilities and their families and taking into account age and culture
  • Ability to respond appropriately to individual needs and priorities
  • Work with limited supervision as well as working as part of a team
  • Demonstrate good organizational skills
  • Practical written and verbal communication skills.
  • An ability to provide a range of personal care and domestic assistance to participants in a way that promotes their independence, values, rights and dignity
  • A reasonable knowledge and understanding of the NDIS Code of Conduct, National Disability standard, WHS, Equal Opportunity Principles and Practices and knowledge of standard safe food handling procedures.
