About the Role
This position is a key member of the Services Team providing assessment, care planning, monitoring and referrals for all clients. A Case Manager is responsible for performing in home face to face assessments and partner with participants, their carers/families to assess current situations and create support plans that enable maximum independence and help participants achieve their goals. The role works specifically with NDIS clients and consists of but is not limited to the following tasks:
- Review intake information, engage new clients, establish the NDIS service agreement, care plan, budget and connect the client with services and supports.
- Work mobile, conducting client visits as needed, practicing person centred care planning, providing independent advice and guidance to clients and their families;
- Work collaboratively with clients, their carers and representatives, ensuring their ongoing needs and goals are supported, to become more independent in their home and in the community;
- Liaise with Home Caring employees, stakeholders, and partnered service providers on behalf of the client and Home Caring;
- Conduct reviews for all clients, revising their care plan, goals, services and budget expenditure as required;
- Ensure operations are conducted ethically, fairly and within the statutory legal and contractual requirements.

Qualifications required for the role
- Tertiary qualifications in allied health or related fields
- In the absence of completed tertiary qualifications, experience in the delivery of person-centred practices in non-complex and/or complex case management and/or demonstrated experience with complex service-oriented offerings in NDIS.
- Diploma in Community Service or social degree is highly regarded.
- RN registered is highly regarded.
