About the role

We are seeking a Community Health Nurse to deliver in collaboration with the Service Manager and Community Outreach Team, an Outreach program within the local government areas of Boroondara, and Manningham. Collaborating with Community Engagement workers.

You will facilitate supportive connections and referral pathways to vulnerable people who are experiencing isolation, marginalised housing or homelessness, and health issues to help them build trusted connections with health services and social activities. This is a Permanent Part time 0.4 EFT position reporting to the Manager, Community Access and Outreach.

What you will be doing

  • Deliver community health nursing services; assessments (including risk assessments), health monitoring and service navigation and supported referrals to residents
  • Support the Outreach team with the direction of the Manager Access and Outreach
  • Internally negotiate smooth referral pathways for the team to support hardly reached clients’ easy access to services
  • Work closely with the Access to Community Program Coordinator and the Access to Community Volunteer Workforce Coordinator to support each other’s programs
  • Reach out to community members outside of the office in creative ways that meet the community needs

What you will bring Key Selection Criteria

  • Registered Nurse Division 1 with registration with current practicing registration with AHPRA
  • Minimum 3 years post-graduate experience preferably within a community setting
  • Demonstrated experience working with people from marginalised and multi-cultural backgrounds
  • Strong interpersonal and communication skills that fosters open honest communication
  • Accredited Nurse Immuniser (Desirable)
