About the role

An exciting opportunity to lead the implementation and expansion of the Care Navigators Program, embedding integrated care navigation services within GP practices across the Eastern Melbourne Primary Health Network (EMPHN).

Lead a Care Navigators team, ensuring best-practice service delivery, operational excellence, and stakeholder collaboration. Scale the program from pilot to full implementation, reaching 60 GP practices by June 2026, while driving best-practice, patient-centered healthcare solutions.

Build strong partnerships with General Practices, EMPHN, and the University of Melbourne to enhance patient outcomes. Join us in transforming primary healthcare through innovative, integrated care solutions.

What you will be doing

Key Responsibilities

• Recruit, mentor, and support a high-performing Care Navigators team, ensuring integration within GP practices
• Manage and expand the service model through staged implementation, meeting operational and contractual objectives
• Build strong relationships with General Practices, EMPHN, and the University of Melbourne to optimize service delivery
• Ensure best-practice clinical governance, compliance, and continuous service improvement
• Oversee budget performance, financial reporting, and resource allocation for program sustainability

What you will bring

Key Selection Criteria

• Bachelor's in Nursing, Public Health, Healthcare Management, or a related field
• Minimum 3 years in healthcare leadership, clinical governance, or service innovation
• Proven ability to lead, scale, implement and expand service transformations in healthcare settings
• Strong interpersonal skills to collaborate with GPs, healthcare partners, and cross-functional teams
• Proven ability to oversee program delivery, track key performance indicators, manage budgets and ensure service excellence
• Skilled in recruiting, mentoring and leading high performing team, fostering engagement and professional growth
