An exciting opportunity to lead the implementation and expansion of the Care Navigators Program, embedding integrated care navigation services within GP practices across the Eastern Melbourne Primary Health Network (EMPHN).
Lead a Care Navigators team, ensuring best-practice service delivery, operational excellence, and stakeholder collaboration. Scale the program from pilot to full implementation, reaching 60 GP practices by June 2026, while driving best-practice, patient-centered healthcare solutions.
Build strong partnerships with General Practices, EMPHN, and the University of Melbourne to enhance patient outcomes. Join us in transforming primary healthcare through innovative, integrated care solutions.
