Igknight is looking for a skilled Client Services Coordinator to join our team.

Within this role, you would have the following responsibilities:

  • Provide individual support to Igknight members (people with disability or ageing), including assessing the nature and extent of disability and implementing support tailored to the individual’s risk assessment and support plan provided by Igknight.
  • Assist in the development of skills, facilitate recreational and social activities, and provide assistance at medical or other appointments.
  • Maintain ongoing monitoring and reporting of members’ progress through detailed notes on the app system.
  • Collaborate with other disability and aged care service providers to ensure quality outcomes for members. This includes coordinating with allied health supports, families, and other carers to deliver integrated support services.
  • Partner with the Igknight managers to conduct member interviews and update care plans and risk assessments on an annual basis or as needed, ensuring these plans remain relevant and effective
  • Offer guidance to members about accessing community organisations or services that align with their goals and aspirations, enhancing their ability to achieve personal objectives
  • Assess members’ needs to develop, implement, and monitor support programs tailored to individual requirements.
  • Support people with disability in adult service units, group housing, and government institutions to foster independent living and well-being.
  • Collaborate with community groups, welfare agencies, and government bodies to address client and community needs.
  • Promote awareness of available resources and services to support clients and their families.
  • Contribute to identifying community needs, developing resource plans, and preparing funding submissions and reports to enhance services.

Skills and Experience

The suitable candidate will have the following:

  • At least one year of relevant full-time work experience (or equivalent)
  • A relevant Diploma or higher qualification
