In2Change Pty Ltd is Tasmania's leading independent Support Coordination business. We are searching for an experienced Support Coordinator to join our team.

Scope of Role / Duties

  • Assist participants to navigate the NDIS system, informing of their rights, responsibilities and choices
  • Assist participants to identify their goals and aspirations and support options to achieve these.
  • Connect participants to resources in a manner that supports inclusion with informal, formal, specialist, community and mainstream services
  • Support participants to develop connections and promote real choice and control in their lives
  • Support participants to explore options, to grow and build their capacity to be as independent as possible
  • Assist through periods of crisis to remain in control and ensure their rights are respected and link with advocacy services as required
  • Provide information and guidance relating to directing their own supports, understanding the NDIS and how to navigate it
  • Assist participants to reflect on what’s working and what needs to change and the provision of best approaches to achieve this outcome.

Selection Criteria

If you have the required experience, skills, knowledge and personal competencies below we look forward to receiving an application addressing your suitability against the criteria below. 

  1. Relevant experience (minimum of 3 years) working in the disability / community sector at a Coordinator / Management experience desirable with demonstrated understanding of the NDIS scheme, the Code of Conduct and commitment to participants choice and control.
  2. Excellent written and verbal communication skills especially an ability to listen, interpret and act on what people want and need in a timely manner
  3. A personal commitment to positive support and Social Model of disability and a strength-based approach to achieving positive change in the lives of people with disability
  4. Demonstrated effective organisational, time management and task management skills are essential and the ability to deal with competing priorities, tight deadlines and putting plans into action
  5. Creative problem-solving skills with the ability to “find a way” and bring people together to achieve outcomes for participants
  6. Motivated and aligned to our purpose, core values and our organisational KPI's including 75% billable income
  7. Ability to work autonomously across a range of settings including home, dedicated office setting and community venues whilst actively contributing to the team culture.
  8. Competency in using cloud-based IT applications including Office 365 including Outlook, Word, Excel and Client Management Systems.
