ABOUT the CENTRE OF EXCELLENCE IN HEALTHY AGING

Australians are living older for longer, but we need to live younger for longer. Improvements in life expectancy are resulting in an ageing population, but are we all living well up until death? The science around prevention of chronic disease and frailty, the enemies of healthy aging, is rapidly evolving. Primary and secondary preventive activities across the aging journey, but particularly in early aging are needed to both improve quality of life and independence in our aging community.

Inala Primary Care, seven primary care services across Australia and several industry partners are creating The National Centre of Excellence (CoE) in Healthy Aging.

VISION AND ACTIVITIES

The overarching vision of our CoE in Healthy Aging is to recognise that aging is a lifelong process. That health and wellbeing in youth, early and middle adulthood determine quality of life and chronic disease burdens in older adulthood. The CoE in Healthy Aging is also designed to position primary care and general practice at the forefront of implementing and testing novel approaches in lifelong prevention and early intervention activities, as well as being the core coordinators of survivorship and end-of-life care for those already impacted by diseases associated with aging. The CoE aims to translate and adapt existing models of evidence-based care, utilising or adapting existing infrastructure to improve the care journey for older Australians and a commitment to sustainable care delivery now and into the future.

The core activities of our CoE in Healthy Aging will be (in order of priority):

• Provide clinical delivery: Immediately translate, adapt and implement novel models of care to sustainably support healthy aging at every life stage and to develop novel tools to support the delivery of this care
• Teach: Provide professional development and education opportunities to other healthcare providers locally, nationally and internationally
• Advocate: Through demonstrating what is feasible and producing outcomes we aim to influence policy and health systems to reimagine models of care and funding to better support healthy aging across the nation
• Research: Have robust evaluation processes in place to measure outcomes and success of the novel approaches in healthy aging and to create new knowledge and evidence of what is feasible and implementable nationally and even internationally. This activity can be achieved through partnerships with academic and industry stakeholders.

Emerging evidence around delivery of care to the aging population and innovation in treatment modalities for age related conditions needs to be translated into delivery across the arc of later life.

position purpose

The Project Manager, CoE in Healthy Aging is a new position that will play a key role in supporting the successful development of our healthy aging project plans as well as contributing to the delivery of the plan in collaboration with the broader primary care community, external partners and stakeholders.

This role provides an outstanding leadership opportunity for a collaborative and strategic thinker who equally enjoys contributing hands on delivery expertise with a broad range of stakeholders. You will enjoy operating at both a strategic and operational level and bringing plans to life. The role will work closely with the Chief Executive Officer, the Clinical Lead and the Project Steering Committee to deliver the core activities of our CoE in Healthy Aging.

key responsibility areas

With the support of the CEO, Clinical Lead and the Steering Committee, the Project Manager will document short, medium and long-term outcomes for the implementation of our CoE in Healthy Aging and develop a detailed initial 12-month project plan. The Project Manager will oversee implementation of the 12-month plan as well as contributing to aspects of delivery including:

• Define project objectives, scope, roles and responsibilities in a detailed project plan to schedule key project milestones and activities
• Define resource requirements and managing resource availability and allocation
• Outline and oversee budget based on requirements
• Identify and engage potential partners and stakeholders to support the delivery of the plan; this may include people and/or organisations across clinical, research, pharmaceutical, corporate, philanthropic and government (state and federal) sectors
• Manage and contribute to delivery of the project
• Organise events and training programs that advocate for healthy aging
• Track project delivery and providing reports on project status to key stakeholders, including achievements, lessons learned, improvements, refinements to scope, timelines and/or budget, tools developed, and training programs created, or other topics as requested through governance
• Develop and maintain a project risk register to ensure identification and mitigation of potential risks
• Manage relationships, engagement and communication with key stakeholders
• Evaluate first 12 months activity against the plan and desired outcomes
• Participate in regular Steering and Clinical Governance Committee meetings
• Contribute actively to meetings
• Support the development of agendas, papers, minutes and actions
• Demonstrate commitment to the CoE’s vision and values
• Abide by the CoE’s policies and procedures (existing and new)
• At all times, maintain confidentiality of information about service users, partners and stakeholders

Essential:

• Strong project management skills, with proven ability to engage diverse stakeholders, meet deadlines, manage budgets and resources and deliver results
• Strategic thinker with the capability to operationalise plans and achieve measurable outcomes
• Skilled in partnership development, identifying growth opportunities and generating support for shared goals
• Creative, innovative and resourceful, with a proactive approach to problem-solving and risk management
• Strong analytical skills and high attention to detail, ensuring accuracy in your work
• Excellent written and verbal communication skills, with an engaging and persuasive style
• Adept at prioritising and managing competing deadlines effectively
• Proven ability to build and leverage networks to enhance collaboration and outcomes.
• Enjoys working collaboratively and flexibly within a small, professional, and geographically diverse team.

Desirable:

• Experience and/or relationships in the health and/or not-for-profit sectors

• Background in clinical care, particularly in the Primary Care, with a commitment to engaging in national health reform and digital healthcare innovation.
• Ability to understand and navigate the complexities of managing the health of an aging population in the primary care context.

eligibility & overview

Status:

This position is full-time, initially for approximately 7 months (ending 30th June 2025), with potential for extension up to six years. It includes a three-month probationary period.

Reporting & relationships:

Reports to the Chief Executive Officer and Clinical lead. Works with Management team, Steering Committee and Clinical Governance Group.

Based at Inala Primary Care, 64 Wirraway Pde, Inala.
