The Supporting Connections Worker ( SCW) works with and identifies Supported Residential Services ( SRS) residents unmet health and social needs and assists them access appropriate services. The role also provides assistance to build the capacity of SRS proprietors and staff. The SCW actively seeks to engage residents in a variety of social and lifestyle activities of their choice .

To star in this role you will have:

• A professional qualification in Social Work or related health discipline
• Experience in working with adults with complex psycho social and health issues
• Demonstrated ability to work effectively as a team member and also as a sole outreach worker
• Excellent knowledge of pension level SRS industry and associated issues
• A clear understanding of client centred and collaborative practice
